In respect of every person enumerated -

(a) full name;

(b) legally valid identification document number;

(c) relationship to reference person, and relationship between members or status within the institution, ship or vessel as be;

(d) sex;

(e) date of birth;

(f) citizenship;

(g) country of birth;

(h) place of ordinary residence.
